Menstruation is a Vital Sign

The menstrual cycle is now often referred to as a “fifth vital sign” for women, and for good reason. The four other standard vital signs include blood pressure, heart rate, temperature, and respiration rate. Medical providers check vital signs because they provide insight into if the body is or is not functioning optimally. Therefore, if a woman is not menstruating monthly that is a key indicator that the body is not functioning normally and further investigation is warranted.

It should be noted that those who are pregnant, on birth control, those who just started their menses, and those who are leading up to menopause are exceptions to the expectation of having a monthly cycle. Otherwise, the menstrual cycle should occur monthly in a predictable fashion.

There are many reasons that women can have irregular menstrual cycles, ranging from hormone imbalances to structural abnormalities. Conditions such as PCOS, hypothalamic amenorrhea, and primary ovarian insufficiency are examples of causes of irregular menstrual bleeding. For women who have irregular menstruation, a thorough medical workup should include a detailed medical history, labs, and in some cases imaging such as an ultrasound.
